Follow that form.  Please do this ASAP.  Next weekend Mr. Miceli will be making the playbill.

If you need examples of what to write for the "Thanks" section, ask.
Here's an example:
"He would like to thank his family, friends, and directors for always supporting him through ever show he has been through."".
